Our Outreach Support service is based on the same principles as our Supported Accommodation provision. Working with the same government departments, we provide support to individuals within their own homes, working to tailormade care plans to achieve their individual goals.
We are very flexible with the support available to clients, from low level support needs to high level dedicated support. Our experienced team will work specifically meet the required outcomes.
We also work closely and have developed partnerships with a number of external services throughout Kent which offer additional benefits to our clients.
